In an effort to start some collection of basic health information we have put together the following survey form. We ask that you complete one of these surveys for each BRT that you own or have owned. We would like to begin a comprehensive study of the variety of health problems that the BRT exhibits and feel that this anonymous survey is a beginning step in this process.
